ABOUT CARECONNECT
- CareConnect has been around since January 1998 and is a partnership between the Voluntary Sector in Merton, Sutton and Merton Primary Care Trust and Merton Social Services.
- In January 2003 the service relocated to Merton Civic Centre, London Road, Merton. While CareConnect is 'linked' to Merton Social Services it is still independent and impartial from Social Services, as its function is to provide solely information and not advice.
- CareConnect's Aims are: To provide an independent public information service and become a focal point for the provision of social, health and community care information for both statutory and non-statutory agencies, as well as, give information on how to access these services. The service is available to personal callers and to people who telephone, write or email in.
Outreach
- CareConnect would be happy to visit organisations or groups in Merton to explain in more detail about the information service and how we can help.
Demonstration Sessions
- If you or a group of colleagues would be interested in a 'behind the scenes' look at how the service actually operates we would be happy to arrange a demonstration session, otherwise, you can view our brief presentation online.
- Demonstration sessions last no more than an hour and can take place at your office or our office - whichever is convenient.
